1ST PHASE:
The student must complete the online registration procedure by clicking here
In order to print the documents needed to start the activities, it is necessary to fill in all the mandatory fields required by the registration procedure and take note of the assigned CODE that will be sent to the email address you indicated during registration. The code will be used, at a later time, to fill out the online questionnaire at the end of the activity and print the final certificate.

NB: Please note that you must be very careful when completing the online registration form, as once you have confirmed your data, it will no longer be possible to modify it.

2 PHASE
After registering online, the student must download and print the PFO, in three copies, so that it can be filled in and signed, first by the school's representatives, then by the university tutor and finally by Uke PASS for the formalization of the start of activities, at the University. The following must be attached to the PFO: a copy of the student's and parents' identity document and the release for video and photo, if agreed.
The method and timing of the activities, at the University, must be agreed upon in advance with the university tutor.

3 PHASE
Once the documents indicated in phase 2 have been delivered, the student will be able to download the following from the “Forms” link:
Logbook, where the activities carried out, day by day, during the entire training course, will be indicated in each box and signed by the student and the university tutor, until the required hours are reached.

4 PHASE
Once the hours of activity foreseen in the training project have been completed, the student must then fill out the online questionnaire which will allow the final certificate to be printed.

Once printed, the certificate must be signed by the School and University tutor and delivered to Uke PASS together with the Logbook to close the procedure.
